Please print and fill out the following forms prior to your first appointment so that Ashlie may adequately assess your medical history, and focus on any body ailments you may be experiencing. 

 

All patients need to fill out the Client Intake Form and Liability Waiver. The Medical Waiver and COVID forms only need to be filled out if required by your doctor or requested by Ashlie.
